Phase Overview

In this training phase, Clinicians focus on providing onboarding documentation necessary for credentialing and paneling. Clinicians will also begin to develop a general understanding of School-based Mental Health and Wellness, recognizing the intersection between healthcare and education systems.

Review Centers of Wellness website

Explore the Centers of Wellness website to become familiar with its content and resources. Identify specific topics that resonate with you and prepare to share insights during the SBMHW Orientation, as this will help facilitate meaningful discussions and enhance your understanding of key wellness concepts.

Complete the necessary documents from the Sacramento County Health Center (SCHC) to move forward in the approval process. This step is necessary to obtain authorization to provide clinical services in schools. Your timely submission of these documents will help expedite your approval.

Credentialing Documents

  • • Complete Sacramento County Health Center (SCHC) VPN form

    • Complete Sacramento County Health Center (SCHC) Application form

    • Apply as a Medi-Cal provider within the Provider Application and Validation for Enrollment (PAVE) system

    • Enroll within the Council for Affordable Quality Healthcare (CAQH) Provider data Portal

    • Apply for National Provider Identifier (NPI) number, if you do not have one already

Compile and complete documents below and submit to designated program analyst for credentialing:

Gather and finalize the required documents listed below, then submit them to the designated Program Analyst for the credentialing process. This ensures all necessary paperwork is in order for proper evaluation and approval.

    ☐   Government-Issued Photo ID

    ☐   Documentation of current PPD results (TB test)

    ☐   Pocket License (LMFT, LCSW, ACSW, AMFT)

    ☐   Individual Practitioner Information Special Training

    ☐   Copy of current certificate of professional insurance (provided by SCOE)

    ☐   Documentation of current Flu Vaccination or sign the Flu Declination Form, and abide by policy

    ☐   Molina HIV and AIDS Specialist Form

    ☐   Anthem Behavioral Health Practice Profile Form

    ☐   Documentation of COVID-19 Vaccine

    ☐   Copy of diploma or transcripts from school where degree was obtained

Onboarding Support

Understand who to contact for support during the onboarding process. Clinicians are given specific administrative points of contact within the program to facilitate efficient communication and support. These contacts are essential for addressing administrative needs and obtaining necessary information.

    • Coordinate with administrative staff to retrieve technology (phone, computer, fob, lock box, badge)
